ATTENTION PARENTS & PARTICIPANTS:
A parent or guardian must complete the form for those 15 and under.  We require a faxed form , by parent or guardian for this age group of participants.  You must be 16 or older to fill out this form.   Due to an overwhelming amount of students wanting to participate we thank you for your cooperation ahead time and we welcome you on board.   We offer discounted rates for youth groups.  You can reach our office at 252-441-3579 for more information.
